Microneedling Aftercare: What to Do, Day by Day
Microneedling works by creating a controlled injury and letting your skin repair it. What you do over the following week decides how well that repair goes — and most of it is about leaving the skin alone.
The treatment itself takes twenty to thirty minutes. The result takes a month. In between is a stretch where the skin is doing something genuinely useful and can be interfered with, so this is the guide we give clients at our clinics, set out day by day.
The first 24 hours
Your skin will feel warm and tight and look flushed, most obviously in the first few hours. It is a sunburn-like flush rather than a rash, and it settles noticeably by the next morning.
Leave it bare. No makeup for 24 hours. The micro-channels are still closing, and foundation is the one thing worth avoiding completely — everything else on this page is a matter of degree, this one is not.
Cleanse gently, or not at all. If your technician applied a serum, leave it to absorb. When you do cleanse, use lukewarm water and something bland, and pat rather than rub.
Skip the gym, the sauna and the hot shower. Heat and sweat on freshly needled skin increase irritation and infection risk. A walk is fine.
Moisturise and protect. A plain moisturiser, and mineral SPF before you go outside. Sun protection starts immediately, not once the redness has gone.
Days 2 to 3
The flush fades. Skin may feel tight, dry or slightly rough — that is the surface layer beginning to turn over.
Mineral makeup is fine from day two. Keep the routine short: cleanse, moisturise, SPF. This is not the moment to reintroduce anything active.
If you had deeper work — scar or stretch mark depths run at 1.0 to 2.5 mm — the redness lasts closer to the full 48 hours and may be accompanied by mild swelling. Both settle.
Days 4 to 7
Light flaking is common here, and it is a good sign. It means the treated surface is shedding and newer skin is coming through underneath.
Do not help it. No scrubs, no exfoliating acids, no picking at flakes, no rubbing with a towel. Pulling skin away before it is ready is the most reliable way to turn an ordinary recovery into a mark that outlasts the treatment by months — and on deeper skin tones that risk is higher, because inflammation is what drives post-inflammatory pigmentation in the first place.
Retinoids and exfoliating acids stay off until around day five. When you do restart, add one product back at a time.
Weeks 2 to 4: where the actual result happens
By now the visible recovery is over and it is tempting to conclude that is the result. It is not.
Collagen remodelling — the process that changes a scar or a line rather than just brightening the surface — runs over roughly four to twelve weeks. This is why sessions are spaced four to six weeks apart, and why judging the outcome after a single appointment tells you very little. A course for texture usually runs three sessions; established acne scarring is planned for six or more.
Daily SPF through this whole period is not a nicety. New collagen forms in skin that is also being asked to repair UV damage if you skip it, and pigmentation that appears now is harder to shift than the concern you came in for.
What to avoid, and for how long
- Makeup — 24 hours
- Exercise, saunas, hot showers, swimming — 24 to 48 hours
- Retinoids, retinol, AHA, BHA, glycolic, salicylic — about 5 days after (and 5 to 7 days before)
- Scrubs and physical exfoliation — until flaking has finished on its own
- Direct sun and tanning — throughout the course, with daily SPF
- Waxing in the treated area — 5 to 7 days either side
- Chemical peels — do not stack them in the same week; space the two treatments deliberately
If you had PRP, PRF or PDRN added
The needling is identical, so the aftercare is too. The single difference is timing: the serum is meant to sit on the skin and absorb, so leave your face completely alone for longer before the first cleanse — usually until the next morning. Your technician will give you the exact window before you leave.
You may also see slight bruising at injection points if PRP or PRF was delivered by injection rather than through the channels. That settles within a few days.
If you had bio microneedling instead
Bio microneedling is a different treatment despite the shared name — silica spicules massaged into the skin, no needles and no open channels — so the aftercare pattern differs.
Expect a warm, prickling sensation for the first day or two, light flaking around the middle of the week, and noticeably fresher skin by day seven as the surface completes its turnover. The same rules about not picking and wearing SPF apply. The restrictions on makeup and exercise are looser, because nothing has been opened.
When to call us
Recovery from microneedling is uneventful for almost everyone. These are the exceptions worth a phone call rather than a wait-and-see:
- Redness that spreads or worsens after day two instead of improving
- Heat and swelling that increase rather than settle
- Any yellow crusting or discharge
- A cold sore developing in or near the treated area

How long does redness last after microneedling?

When can I wear makeup after microneedling?
Leave the skin bare for the first 24 hours. The micro-channels close over during that period and putting foundation into an open channel is the one avoidable way to cause a problem. From the next day, mineral makeup over SPF is fine.
When can I go back to retinol?
Stop retinoids, retinol and exfoliating acids — glycolic, salicylic, AHA and BHA — five to seven days before your appointment, and leave them off for about five days afterwards. Reintroduce one product at a time rather than restarting a full routine at once.
Can I exercise after microneedling?
Give it 24 to 48 hours. Heat and sweat on freshly needled skin increase both irritation and the risk of infection, and a hot gym or a sauna is the worst version of that. Walking is fine the same day.
Is peeling or flaking normal?
Light flaking around day two to four is normal and means the surface is turning over. Do not help it along — picking or scrubbing flaking skin is how a straightforward recovery turns into a mark that lasts months. Let it come away on its own.
What is different about aftercare with PRP or PDRN?
The aftercare is essentially the same, because the needling is the same. The one difference is that the serum is meant to sit on the skin and absorb, so leave your face alone for longer before cleansing — usually until the following morning. Your technician will tell you exactly when.
How is bio microneedling aftercare different?
Bio microneedling uses no needles, so there are no open channels and the rules are gentler. The pattern is different too: expect a warm prickling sensation for a day or two, light flaking around the middle of the week, and fresher skin by day seven as the treated surface completes its turnover. Sun protection still matters.
When should I contact the clinic?
Spreading redness that worsens after day two rather than improving, heat and swelling that increase rather than settle, any yellow crusting or discharge, or a cold sore developing in the treated area. None of these is common. All of them are worth a phone call rather than waiting to see.
Before your next session
The preparation rules mirror the aftercare ones: retinoids and acids off five to seven days beforehand, no waxing, no sunburn or fresh tan, and let us know if you have had a cold sore in the area. Active breakouts are a contraindication in our protocol, so tell us if your skin has changed since the last visit.
